De plus en plus, la santé publique constitue une préoccupation majeure de l’opinion et des autorités politiques. En témoignent l’attention apportée à la lutte contre le handicap (dont fait partie les drépanocytaires homozygote SS, les albinos et les personnes de 3ème âge), l’insécurité routière et le cancer, déclarés chantiers pour le quinquennat par le président de la République.

La drépanocytose ou anémie falciforme est une hémoglobinopathie   la   plus   fréquente   au   monde,  survient essentiellement   chez   les   sujets   de   race   noire   et   dans   les   populations   du   pourtour Méditerranéen. La RDC est le 3ème pays les plus touchés au monde après le Nigéria et l’Inde. Il   s'agit   d'une   maladie   génétique   caractérisée   par   la   présence d'hémoglobine   S qui détruit les globules rouges.

En RDC, 35% de la population est drépanocytaire hétérozygote AS et 2% est homozygote SS. Ainsi, un individu ne peut donc être atteint que si ses deux parents lui ont transmis l'allèle (S) responsable ; il est alors dit homozygote (S/S).

Elle entraîne des crises douloureuses et des troubles vaso-occlusifs, signes de graves hémolyses donc, l’anémie. Qui dit anémie dit mauvaise tolérance aux efforts physiques puisque la fatigue, induite par l'anémie, est permanente mais elle est aggravée par les efforts. Ces efforts demandent de l'oxygène et ceci entraîne une fatigue plus importante. Ce qui pose le problème d’insertion psychosociale et la RDC étant un pays en voie de développement, il n’y a pas une bonne prise en charge.

Autrefois 80% des homozygotes (HBS//HBS) mouraient avant l'âge de la reproduction. Aujourd'hui, grâce au dépistage précoce, à la prévention des infections (vaccination, antibiothérapie systématique), à la prévention de la déshydratation et de tout autre cause pouvant la provoquer, la maladie reste sérieuse et invalidante mais l'espérance de vie s'est considérablement normalisée.

Pourtant, même si les arguments en faveur d'une bonne prise en charge des drépanocytaires se multiplient, la lutte contre la drépanocytose n'est pas toujours une priorité pour les pouvoirs publics. C'est notamment le cas de la RDC, où les budgets insuffisants des services de santé sont avant tout consacrés à la lutte contre d’autres maladies graves comme la tuberculose, la malaria et le VIH/SIDA.

Dans cette perspective, le présent rapport s’efforce de partager les expériences sur la drépanocytose avec les drépanocytaires et leurs membres de famille, les parents en particulier pour qu’ils s’informent et s’impliquent à la lutte contre cette maladie.

L’activité a été organisée en date du 23 Mai 2016 à l’Hôtel INVEST.

2. Les éléments de suivis d’un drépanocytaire par Dr. Ange NGONDE

Notre bataille est de découvrir tôt la maladie afin   de se battre pour la survie de cette enfant. On soigne des symptômes pour améliorer la qualité de vie de la personne.

  • Education thérapeutique des parents sur les facteurs déclenchant les crises ;
  • Les signes d’alerte se soignent à l’Hôpital ;
  • Les règles d’hygiènes ;
  • Les indicateurs de suivi (5) :

 

  • Absentéisme ou rendement scolaire ;
  • Respect des règles d’hygiènes ;
  • Surveillance de bilan de routine (Hb, GB, FL, GE, …) ;
  •  Tenue du dossier transfusionnel ;
  • Bilan de détection des complications.

Elle nous a sensibilisés puisque la drépanocytose nous concerne tous. Si pas directement, par un membre proche.

On doit s’accepter, que cette maladie ne soit pas une fatalité mais un moyen de vivre longtemps. Il faut une prise en charge globale et intégrale : accompagnement psychologique, médicament et nutrition.

Plus de 90% des maladies s’accompagnent de la production des substances toxiques. Les aliments consommés (légumes, fruits, …) peuvent nous protéger contre ce phénomène de production des substances toxique produite en cas de drépanocytose et diminuer leurs effets néfaste appelé stress oxydatif.

 

Pour prévenir les crises, consommons plus des aliments qui contiennent des antioxydants :

  • Sélénium qu’on retrouve dans le Lentinus (Bulongo) ;
  • Zinc dans Auricularia delicata (Kilebu, Matoyi) ;
  • Vitamine C (Bilolo, Pondu) ;
  • Vitamine E (Kikalakasa, Oseille) ;
  • Lumbalumba , Bulukutu améliorent la forme des Globules Rouge ;
  • Niebe (mbuengi), Moringa.

C’est lorsqu’il y a une situation négative, on ne capitule pas mais on le met en défi. Quand une situation vous met dans une position de faiblesse devient une force. Cet enfant devient plus grand, plus beau, plus fort. Les parents doivent mieux se connaitre pour aimer autrement leur enfant.

Nous sommes une force, on peut se mobiliser et lutter contre cette maladie. Nous sommes les sans souci (SS), on a quelque chose que les autres n’ont pas.

Les parents ont un défi d’aller au delà de leurs souffrances. Toute situation négative est une occasion en or pour apprendre surtout quand on est jeune.

Etudiant en médecine (médecin stagiaire). Quand on s’accepte, Dieu va nous aider. Dans la vie, tous les chemins sont bons tant qu’on ne recule pas.

Le vieux drépanocytaire qui a inspiré les jeunes comme Jean Gabriel à devenir médecin. Il a vécu la résilience en transformant sa maladie en état de force. Il fût le seul à faire une publicité pour crier tout haut qu’il est drépanocytaire alors que les parents cachaient la maladie de leurs enfants. Il ne faut pas avoir honte de sa maladie mais la transformer en force.

9. Intérêt du diagnostic moléculaire dans la drépanocytose par professeur MIKOBI MINGA

  • Le pronostic est plus favorable si on prend en charge précocement le malade.
  • Le diagnostic se fait par le programme de dépistage néonatale ; car le diagnostic tardif signe une complication qui a révélé la maladie (CVO, infection, anémie) ce qui a entraine une mortalité infantile (les enfants SS meurent avant l’âge de 5ans).

Il existe plusieurs techniques pour faire le dépistage néonatal :

  • La technique de l’IEF (iso-électro focalisation) pour le dépistage de masse ; analyse 60 à 70 échantillon qui identifie les personnes à risque suspect. En RDC, le professeur TSHILOLO a mené une étude en 2009 de dépistage néonatal par cette technique ;
  • L’électrophorèse capillaire ou HPLC qui sépare toutes les fractions de l’hémoglobine associées à l’hémoglobine S et nous révèle quel enfant développera la maladie ;
  • Le test rapide biochimique qui migre dans 3 minutes ;
  • Le diagnostic moléculaire met en évidence la mutation E6V du chromosome 11 par l’extraction de l’ADN à partir des globules blanc puis la réalisation de l’examen par la technique PCR-RFLP.

Les avantages du diagnostic moléculaire:

  • C’est un diagnostic de confirmation par excellence ;
  • La possibilité de faire l’haplo typage ;
  • N’a pas besoin d’attendre 3 mois après la transfusion ;
  • A la possibilité de faire un diagnostic néonatal et anténatal ;
  • Permet la ponction et l’examen pour le diagnostic.

V. RECOMMANDATIONS ET PERSPECTIVES D’AVENIR

  • Le PNLCD crée par l’ancien ministre de la santé, le professeur MASHAKO, n’a pas un programme de dépistage ; Mettre en place un programme de dépistage néonatal.
  • Rendre ce test moléculaire disponible et accessible.
  • La drépanocytose reste encore dans beaucoup d’ethnie une maladie liée aux croyances, aux esprits et au mauvais sort ; mobiliser les ressources pour informer et conscientiser la population par des sensibilisations.
  • Eliminer la pauvreté car en Afrique, la drépanocytose reste encore une maladie des pauvres.
  • Plaidoyer pour que les soins drépanocytaires soient subventionnés : revenue annuel inférieur à 800$ alors que les soins annuels d’un enfant drépanocytaire est de 800$.
  • Equiper les laboratoires.

VI. CONCLUSION

Sommes toute, la drépanocytose est une maladie compatible à la vie, le Rézodrépano SS doit s’organiser d’avantage pour faire connaître cette maladie par des sensibilisations et la bonne prise en charge afin que les drépanocytaires ne se cachent plus, mènent une vie de qualité et que tout le monde s’impliquent à lutter contre cette maladie.
